Final report to Pharmaceutical Health and Rational Use of Medicines Committee

         This project seeks to further the implementation of the APAC National Guidelines to achieve the continuum of quality use of medicines between hospital and community. It involves the South Eastern Sydney Area Health Service and the local Divisions of General Practice (Sutherland, St George, South Eastern Sydney and Eastern Sydney). Clinical review of medications following discharge is a key focus of this strategic alliance. The audit process yielded 203 patient questionnaires from 106 GPs across the four Divisions of General Practice.
