As
part of the Government Action Plan for Health, the NSW Government last
year committed $45 million over three years to improving chronic disease
management in the State. Cardiovascular disease and its risk factors
(including diabetes), respiratory illness and cancer were identified as
three priority program areas, and the broad goals established for these
areas were to:
prevent crisis situations and reduce urgent unplanned admissions to
hospitals for people with the nominated chronic conditions;
improve the quality of life of these people; and
improve the quality of life of their carers and families.
Under
this framework, South East Health was allocated $2.2m over three years.
This was subsequently assigned to seven programs, for which the Area
submitted proposals late last year. A brief overview of the programs,
