| Abstract: |
Aged
Care services in South East Health will be provided on the basis of four
geographical sectors and there will be a coordinating Aged Care Strategy
group. In each sector there will be management teams who will have
consumer advisory groups and intersectoral meetings which will involve the
four local Divisions of General Practice. Information systems will be
developed to enhance integration of care across the hospital-community
interface. The future service commitment will be to improving the care of
older people in our general services as well as the development of new
special services dedicated to older people and their carers. |
